How to Apply for Long-Term Care

Admission to all long-term care homes in Ontario is co-ordinated through the Community Care Access Centre (CCAC). To contact the CCAC in Waterloo Region, call 519-748-2222 and ask for the Intake Department.


A CCAC case manager determines eligibility for long-term care based on the applicant's health care needs. Once a person is declared eligible, their name is placed on the waiting lists of as many as five long-term care homes.

*Permanent residents in basic accommodation may apply for a reduced rate. They must supply a copy of the previous year's Revenue Canada Notice of Assessment or current income documentation. To be eligible they must have applied for all available benefits before applying for a Rate Reduction.

How it works:

An individual's room rate is based on his or her income alone, not his or her family's income. If a resident applies for a reduced rate, assets are not considered in determining if a reduction is appropriate. The formula for rate reduction is intended to leave the resident with $143 per month for personal use.
